Podluzhnyi O. Differential Approach to the Surgical Treatment of the Patients with Adrenal Tumors

The thesis is aimed to improve the results of treatment of patients with adrenal tumors by working out a differential approach to the surgical treatment. The thesis analyzes the diagnosis and treatment of 100 patients with adrenal tumors. The laboratory and CT-scan characteristics of different kinds of adrenal tumors are presented. It is proved that adrenal vein sampling is the main method in differential diagnosis of unilateral or bilateral aldosterone hypersecretion of the patients with primal hyperaldosteronism. The indications for surgical treatment of patients with adrenal tumors are confirmed. The advantages of endoscopic adrenalectomies over open operations are proved by statistical methods. A decrease of postoperative complications and hospital bed-day is found in group of patients who passed through endoscopic treatment (р<0,05). The indications for endoscopic adrenalectomies are extended. An endovascular adrenal destruction is suggested to the patients with primal aldosteronism and adrenal hyperplasia. We worked out an algorithm of diagnostic and treatment of patients with adrenal tumors, based on our own experience.
