The work is dedicated to optimizing the treatment of patients with coronary artery disease and postinfarction cardiosclerosis after CABG surgery using ACE inhibitors and angiotensin receptor blocker. The thesis is based on a comprehensive study of 170 patients with coronary artery disease and postinfarction cardiosclerosis after CABG surgery, including 105 patients received standard therapy and ACE inhibitors (enalapril, lisynopril), and 65 patients - angiotensin receptor blocker (valsartan). The influence of angiotensin receptor blocker in a dose of valsartan 160-320 mg/day on the prevention of arterial graft stenosis, myocardial improvement in LV function, remodeling of the left ventricle, central hemodynamics and left ventricular preload and in preventing of the of ventricular arrhythmias development was proven. Also was shown, that to achieve the target average daily blood pressure profile in patients with coronary artery disease and postinfarction cardiosclerosis it is advisable to appoint valsartan 160 mg twice a day.
