The dissertation is devoted to improvement of methods of clinical supervision for preterm infants born weighing less than 1250 grams, based on a comprehensive analysis of the clinical, instrumental, immunological parameters and evaluation of them as early markers of short-term and long-term adverse outcomes of perinatal pathology in this category of children. The study identified features of diseases and causes of death in preterm infants in the neonatal and post-neonatal period before reaching corrected age of 18 months, the expediency of the use of standardized scales to assess psychomotor and social-emotional development was proved, the algorithm of early hearing screening, was developed, immunological predictors of neonatal death, disability and developmental delay were established. It is shown that early markers of neonatal death or disability in the corrected age of 18 months are the content of toll-like receptors type 2 and proinflammatory cytokine interleukin-6. Based on identified clinical and paraclinical features and applied diagnostic strategies, the follow-up scheme for very preterm children was improved, thus reducing the incidence of adverse long-term outcomes, such as cerebral palsy and post-neonatal death, in premature infants born weighing less than 1250 grams.
