Shklyaeva O. Peculiarities of ischemic stroke clinical course in patients with atrial fibrillation

The thesis is devoted to the study of disturbances of blood pressure circadian rhythm in acute period of ischemic stroke in patients with atrial fibrillation. Clinical risk factors of ischemic stroke in patients with atrial fibrillation were defined. Clinico-neurological peculiarities of acute period clinical course of ischemic stroke in patients with atrial fibrillation were defined, which are characterized by intensity of some neurological syndromes and severe functional disorders. In acute period of ischemic stroke in patients with atrial fibrillation according to the data of daily monitoring of blood pressure the development of night hypertension, increasing of mean daily diastolic blood pressure and morning increasing of diastolic blood pressure have been detected, which are considered to be risk factors of unfavorable clinical course of acute period of ischemic stroke. Structural and functional cardiac indexes which favor development of circadian rhythm of blood pressure "night-peaker" were defined. Based on the results of the investigation recommendations for treatment patients with acute ischemic stroke and atrial fibrillation have been developed and their effectiveness has been established.
