The dissertation is devoted to development of brand new functional conservative-orthopedic method of treatment of mandibular fractures within its dentition by development of compression-distraction appliance (CDA) with two screw pairs on a distance. With the help of biomechanical researches intraoral CDA has been developed, which allows additional correction of position of jaw fragments to create optimal conditions for healing of bone injury. Indications and contra indications have been defined. Biomechanical researches of constructional characteristics of CDA in the appliance -tooth-bone system and comparative analysis of reliability of mandibular bone fragments fixation in its fractures with various constructions were made. Comparative evaluation of treatment using CDA, mandibulomaxillary splitting and a smooth splint bracket was made. It is proved that the proposed method allows to decrease the complications up to 3.3 % (bone fragments shift), while when using mandibulomaxillary splitting method complications make 18.4 % (inflammatory - 5.2 %, secondary bone fragments shift - 13.2 %), and 23,3% complications for smooth splint bracket splitting (3,3 % and 20% accordingly). This comparative evaluation of treatment of patients is based on electromyography of masticatory muscles and Doppler of mental foramen. It is proved that developed system of dental fixation of two screw pairs allows improving functional results and reducing the time of treatment.
