Object - acute pancreatitis (AP); goal - to improve the results of treatment of patients with AP by examining the role of endothelial dys-function and microcirculatory disturbances in the development and prognosis of severe disease, the development of active corrective action within the concept of continuity pancreatonecrosis; methods - clinical, radiological, endoscopic, ultrasound, computed tomography; Laser Doppler flowmetry, biochemical, statistical; results - first studied changes in the functional state of the vascular endothelium and microvasculature in patients with AP and installed early disturbances in the microcirculation and symptoms of reperfusion syndrome in the early treatment of patients; found an association between hemodynamic disturbances, endothelial dysfunction and the system of free radical oxidation; proved that the degree and nature of the violations of the functional state of the endothelium of blood vessels and changes in the microvasculature is closely correlated with the severity of the AP; established that the low activity of antioxidant protection and vascular endothelial likely development of severe pancreatitis; pathogenetically substantiated scheme early conservative therapy for the prevention of severe pancreatitis; branch – medicine.
