Essay

The present work is devoted to the solution actual problem of modern oncology - the evaluation of clinical relevance of tumor cells immunohistochemical profile as marker of serous cancer heterogeneity for the assessment of malignant degree and individual prognosis. This work is performed on clinical dates of 124 patients with ovarian cancer (OC) IА-IIIВ stage (retrospective study). In the study were used the clinical, morphological, immunohistochemical, statistical methods. Patients have been not treated with neoadjuvant chemotherapy. The systematization of cytomorphological signs of tumor growth and histoarchitectonic peculiarity of tumor structure were conducted and two grade of OC histologic malignancy - high (63,7% patients) and low (36,3% patients) were defined. It was investigated the connection of OC immunohistochemical profile (expression of Кі-67, VEGF, CD44s, Е-cadherine) with histological malignancy of identical tumor stage and differentiation that indicates the serous cancer heterogeneity. It was shown that high grade of histologic malignancy and Кі-67, VEGF, CD44s high expression, and E-cadherine low expression are the criterions of unfavourable individual prognosis that were confirmed by indexes of 5-year survival. The role of adhesion of tumor cells in ovarian cancer progression, heterogeneity expression of CD44s expression in different parts of the tumor, including some clusters (budding) were established. Wide range of morphological changes in the contralateral ovary, uterus and fallopian tubes of patients with stage I OC were discovered that are the morphological reflection of hormonal homeostasis disturbunce. The obtained results of a comprehensive study can be the basis for determining the histologic and biologic OC malignancy, individual prognosis of tumor processes and correction of adjuvant treatment.
