The thesis is devoted to optimize the treatment of patients with arterial hypertension with obesity and disorders of the dopaminergic exchange by exposure correction of disorders of the dopaminergic system agonists of DR, antagonists of aldosterone receptor and antagonists of calcium. A comprehensive survey of 151 patients with arterial hypertension and obesity showed inhibition of the dopaminergic system on the background of hyperactivity of the renin-angiotensin-aldosterone system (RAAS). The more pronounced changes are observed with concomitant obesity that is associated with lipid disbalance, disorder of the vasodilatating endothelial function, violation of the geometry of the heart and the deterioration of the diastolic dysfunction. Clarified the influence of the dopamine on the RAAS, a metabolism of lipids, the data of the 24-hour blood pressure monitoring, endothelial function and structural-functional state of the heart. The effect of different pharmacological schemes of antihypertensive treatment in patients with obesity and impaired dopamine metabolism on the clinical course, the structural-functional state of the myocardium, endothelial function, the data of RAAS and dopamine metabolism, by the addition to the therapy agonists of DR bromocriptine and pramipexole.
