Essay

The thesis presented theoretical generalization and new solution of scientific and practical problems of cognitive impairment in patients with hypertensive encephalopathy. The study highlighted the following key findings: Refined pathogenetic mechanisms of cognitive dysfunction based on a comprehensive study of clinical and neurological data, results of instrumental methods, indicators daily blood pressure established during clinical course, and improved diagnostic methods. Established clinical risk factors for cognitive impairment in patients with hypertensive encephalopathy. Proved significantly progressive course of atherosclerotic and hypertensive encephalopathy. Defined parameter of neuropsychological testing, brain perfusion, and indexes of daily profile of blood pressure are associated with the development of cognitive impairment in patients with hypertensive and atherosclerotic encephalopathy. An analysis of the circadian blood pressure profile revealed a decrease in daily index of systolic and diastolic blood pressure that was accompanied by a deterioration of neuroimaging findings and neurological symptoms. Examination of textural properties should be an integral part of the evaluation of SPECT imaging in patients with hypertensive encephalopathy.
