The given thesis deals with the efficiency improvement of a complex intensive therapy in pregnant and parturient women with severe preeclampsia by correcting an antihypertensive therapy followed by evaluation of its effectiveness.The existing methods of intensive care of pregnant and parturient women with severe preeclampsia due to early application in addition to the standard protocol of treatment of drugs with a peripheral and central vasodilating effect have been improved. The algorithm for the central hemodynamics parameters monitoring and the cerebrocardiac interactions, allowing to evaluate objectively the preeclampsia severity has been worked out. The hemodynamic profile type, the centralization degree of the cerebrocardiac interactions, the level of the CNS functional state and the adequacy of the compensatory-adaptive reactions of pregnant and parturient women with the severe preeclampsia have been detected with the use of a heart rate variability methods, omegametry and rheography with the study of central hemodynamic parameters. According to HRV (heart rate variability) and omegametry data, the fixation of the hemodynamics and cardiac rhythm dysregulation during the transesophageal echocardiography is a result of the inhibitory control relief and deficiency of the general integrative control of the nervous system. The altered fetoplacental complex serves as the stimulus for the formation of the pathologically enhanced excitation generator that creates a pathological determinant as a system forming mechanism and the key link of the pathological system during the transesophageal echocardiography.It calls for further researches in the field of pregnant women preeclampsia diagnosing and intensive therapy, as well as the selection and introduction of new up-to-date medications, aimed at improving the quality of treatment and prevention of possible complications of pregnancy and childbirth.
