The examination and treatment results of 200 patients with acute varicothrombophlebitis of great saphenous vein. The growth of the thrombus is influenced by the following hemodynamic factors: collateral outflow, spread reflux in the deep venous system with insufficient perforative veins, refluxes interaction, forming of vicious circle of venous circulation. The threat of a thrombus translocation into deep veins through insufficient perforative in acute varicothrombophlebitis of GSV makes up 12.2 %. Therefore, urgent surgery should be carried out in case of the direct threat of pulmonary embolism (embologenic thrombus) or in the case of thrombus translocation to the deep veins (thrombosed perforative vein, the thrombus top is 5 сm or less to the junction) or for the pulmonary embolism prevention - atypical forms of the acute varicothrombophlebitis of great saphenous vein, the reflux reaches the top of a thrombus. In all other cases, the delayed intervention are recommended.
