The dissertation is devoted to development of an integrated system of therapeutic measures on the basis of investigations of clinical-psychological peculiarities in patients with different levels of breach of medication compliance (MC) in depressions. An investigation of individual-personality peculiarities and the sphere of needs and motivations showed predominance of accentuated affective-rigid and cyclothymic personality peculiarities with leading needs for an active life and social-psychological dysadaptation with self-non-acceptance among patients with lack of MC and partial MC in depressive episode. In patients with a complete MC in this pathology pedantic personality peculiarities with leading needs in the health sphere and social-psychological adaptation with self-acceptance were found out. Lack of and partial MC in recurrent depression were associated with excitatory personality peculiarities with leading needs for material well-being and social-psychological dysadaptation in the social-occupational sphere, self-non-acceptance and non-acceptance of others. In patients with a complete MC in recurrent depression anxious personality peculiarities with predominated need in the health sphere, social-psychological adaptation and acceptance of others prevailed. On the base of data obtained it was worked out and implemented an integrated system of therapy for depressive disorders accompanied by MC breaches. Measures of the system are directed to the main factors promoting breaches of MC (factors associated with disease, with personality of the patient, with medication, social-psychological and healthcare organizational factors).
