Object - incisional hernias in obese patients; goal – improve outcomes in patients with incisional hernia and concomitant obesity, due to the development-stage surgical treatment strategy using modern bariatric surgery; methods – clinical, laboratory, instrumental, statistical, morphological; results – for the first time on the basis of morphological study found that obesity leads to morphological changes of the musculo-aponeurotic structures of the anterior abdominal wall in the form of degenerative changes in its structural elements and their replacement by loose connective and incisional tissue, which promotes the formation and recurrence of incisional hernias; for the first time on the basis of a comparative analysis of results of surgical treatment of patients with scar hernia and concomitant obesity proved the efficacy of bariatric surgery as the first stage of the treatment of the above mentioned category of patients; for the first time on the basis of evidence-based analysis using the statistical methods of processing identified the advantages and disadvantages of two- and one-stage approach on the basis of which the developed diagnostic and therapeutic algorithm for treatment of patients with c incisional hernia with concomitant obesity; improved known method of hernia repair and method of surgical treatment of obesity; branch – medicine.
