Object of study - chronic anal fissures; аim - to improve the immediate and long-term results of complex surgical treatment of patients with complicated forms of chronic anal fissure through the study, development and implementation of new methods of preoperative preparation and radical surgery; мethods - clinical, laboratory, instrumental, general and special histochemical, cytological, morphometric, microbiology, statistics; results - reasonability of application of direct current electric field and etiopathogenetically substantiated medical means is scientifically proved for pre-operative preparation for the first time; a new method of surgical treatment of complicated chronic anal fissures is substantiated, designed and introduced into practice; for the first time generic content and population level of the anal microflora and chronic anal fissure and its sensitivity to antiseptics are studied; the notion concerning pathogenesis of chronic anal fissure as a consequence of anodermal colonization by microorganism associations is widened; the role of microbe biofilm in etiopathogenesis of complicated chronic anal fissures is proved; for the first time the optimal parameters for fissure tissue excision with surgical treatment are substantiated; the role of free radical protein oxidation in the tissue of chronic anal fissure is indicated for the first time as an important factor for the course of wound process; the functional state of basal keratinocytes in the area of fissure excision is estimated for the first time which reflects the functional epidermal condition of the operation borders by means of detection of variation coefficient of optic density of the nuclear colouring and specific square of basal keratinocytes with euchromatization of the nucleus in 1 mm2.
