Liashenko M. Improving methods ofcomplexenteral therapyin the surgicaltreatment of patients withpurulent-inflammatory diseases of the abdominal cavity

This thesis is about improving the results of treatment of patients with purulent-inflammatory diseases of the abdominal cavity using a differentiated approach to the choice of the treatment strategy that includes the correction of enteral insufficiency syndrome with improved methods of enteral therapy. The Research was carried out among 2 groups of patients in order to develop and assess the effectiveness of the method of correcting the syndrome of enteral insufficiency for patients with generalized peritonitis. A new method of enteral therapy that involves a new enterosorbent (diosmectite based on the Crimean blue clay) was developed. It was found that the analysis of anti-LPS-IgA, anti-LPS-IgM and anti-LPS IgG indicators allows us to determine the intensity of a specific anti-endotoxin immunity, judge indirectly about severity of bacterial translocation in the intestine and evaluate the effectiveness of methods used for insufficiency syndrome prevention and elimination. It was found that conducting the preoperative preparation (which ensures inactivation and effective clearance of endotoxin from the intestine) for patients allows to reduce the risk of formation of inflammatory complications by 3,1 times ( 2 = 4,842; p <0.05) and by 2,1 times ( 2 = 3,844; p <0.05) in the 1st and 2nd groups respectively.
