Essay

The thesis is devoted to the study of clinical, functional and pathophysiological features of secondary dilated cardiomyopathy (DCM) of professional, alcoholic, ischemic and mixed genesis. It was established that secondary DCM clinic differs depending on the etiological and pathophysiological factors. Dilatation of the right ventricle occurs more often in patients with alcoholic DCM and dilatation of the left ventricle occurs more often in patients with professional DCM. The highest dilatation of the heart chambers and the lowest ejection fraction were diagnosed in patients that have dilated cardiomyopathy of mixed genesis. The characteristic ECG signs as dilation of the left and right ventricles, which allows to suspect and diagnose secondary DCM were revealed. The relationship between DCM form and microalbuminuria figures was shown: the lack of protein in the urine was often observed in patients with dilated cardiomyopathy of alcoholic genesis, and least likely in patients with DCM of mixed genesis. It was revealed that secondary DCM course is accompanied by systemic inflammation, manifestation of which is the significant increase of proinflammatory cytokines, with the highest rates observed in patients with alcoholic form of dilated cardiomyopathy.
