Watseba T. Correction of insulin resistance in patients with primary hypothyroidism under conditions of iodine deficiency

Object of the study: hypothyroidism, treatment of patients with primary hypothyroidism, obesity primary disorders of carbohydrate metabolism. The aim of the study: was to increase the effectiveness of treatment of patients with primary hypothyroidism with obesity and primary disorders of carbohydrate metabolism under conditions of iodine deficiency on the basis of studying the dynamics of neurohumoral and biochemical parameters, cytokine imbalance and manifestations of insulin resistance (IR) in hypothyroidism. Research methods and equipment: General clinical, biochemical, immunoassay, instrumental, statistical; an ultrasound machine, "TOSHIBA", analysers Stat Fax 303+, BIO-RAD D-10, Humma Star 600. Theoretical and practical results: the carried out set of studies to determine the biochemical, neurohumoral indicators of cytokine profile give grounds to diagnose the state of IR in patients with hypothyroidism and to determine the advisability of the appointment of Metformin in patients with obesity and primary carbohydrate metabolism disorders to correct dysglycemia, reduce visceral obesity and insulin resistance. This approach helps to improve the diagnosis of metabolic disorders, and effective compensation of hypothyroidism, normalization of carbohydrate and lipid metabolism, and lower IR and risk of cardiovascular morbidity and mortality. Novelty: Designed and implemented a new method of diagnosing IR in patients with hypothyroidism. Direct dependence of IR, power immunomodelling reactions of the body and indices of cardiovascular risk from a form of hypothyroidism and body mass index (BMI). Proposed new approaches to the examination of patients with hypothyroidism and treatment with the appointment of Metformin in patients with obesity in primary carbohydrate metabolism disorders and insulin resistance. Level of implementation: the research results are embedded into clinical practice of specialized medical institutions of Ukraine, and also in educational process of medical colleges. Field of application: endocrinology. Degree of introduction: the practice recommendations have been used in clinical practice of department of endocrinology of the Ivano-Frankovsk regional clinic hospital and the Central district hospital. The main results of study have been proposed for use in the educational process of the department of endocrinology of State institution “Ivano-Frankovsk National medical university ”.
