Essay

The thesis presented scientific substantiation and ways of solving the actual task of clinical oncology - optimization of breast cancer treatment with high risk of decease recurrence of neoplastic process. The connection between progression of disease in patients with breast cancer of the stage T1-4N0-2M0 and presence of DTC in BM, high level of TNF and CSF-1 in BM and PB were detected. In accordance with the indexes of presence / absence of DTC in BM and the level of the necrosis factor in BM and PB were formed groups of patients with low (status DTC?TNF?) and high risk of disease recurrence (status DTC+TNF+, DTC?TNF+). The optimal differential medicamental treatment codes of the breast cancer patients with different risk of recurrence were developed, for example, it is necessary to use preoperative maintenance chemotherapy according to the code AC?P with zoledronic acid during postoperative period for the patients with high risk of disease progression; and we consider that application preoperative maintenance chemotherapy according to the code AC as optimal for the patients with low risk of disease progression. It was shown that carrying out reconstructive- restorative operations in breast cancer patients can be integrated into a comprehensive treatment of breast cancer.
