Essay

On the basis of the examination and treatment of 180 patients with pigmented skin tumours developed and introduced into clinical practice new methodological approaches and technologies to optimize the diagnosis of melanoma, melanoma-dangerous nevi and pigmented nevi. Diagnostic usefulness of spectrophotometry and digital images of skin melanomas and pigmented nevi were established. It was revealed the indirect relation between tumor pigmentation of the skin and the spectral chart of the red channel in the digital image. Improved algorithm for the diagnosis of patients with pigmented lesions of the skin was elaborated. Based on the results of spectrophotometry of digital images of pigmented tumors, differential-diagnostic criterias of melanoma, melanoma-dangerous nevi and common pigmented nevi were installed.Based on the results of spectrophotometry of digital images of pigmented tumors, were identificated the criterias for the differential diagnosis of skin melanoma: Іstart ? 18,5 units, Іmax ? 115,5 units, AUC128 ? 5442,1, melanoma-dangerous nevi: Іstart > 18,5 units, Іmax > of 115.5 units, AUC128 <5442,1, and common pigmented nevi: Іstart ? 56,5 units, Іmax ? 125,5 units, AUC128 ? 3706,8.Comparison of histological, spectrophotometric, ABCD clinical diagnostic criterias suggests that the analysis of digital images of tumors significantly improves, compared with the ABCD test, the quality of pigmented skin tumors diagnosis at the pre-hospital period (sensitivity SPМ- 95,5%).
