Smolianyk K. Prophylaxis of bronchial stump failure after lung resection.

The dissertation presents the research results of the study of 149 patients who had complete or partial removal of the lung was performed. The work investigated the nature of morphological changes in bronchial wall depending from accompanying disorders that affect the risk of bronchial stump failure after lung resection. The study developed a highly sensitive predictive risk assessment system of insolvency bronchial stump. Differentiated approach has been developed to the prevention of bronchial stump failure to the extent that the risk of this complication. It has been developed and applied a method of prevention of surgical failure bronchial stump, comprising a two-stage sealing stump using allografts from regenerated cellulose. The work developed and applied a temporary obturator and minimally invasive method of closing post-resection bronchopleural fistula. It has been proved the effectiveness of the proposed methods of prevention of bronchial stump failure and surgical treatment of post-resection of pleural empyema with bronchopleural fistula, based on the analysis of treatment results of patients who had resection of the lung were made.
