Object - dysfunction of the bowel in patients with surgical diseases; aim - to improve treatment results of surgical patients by research of the causes and mechanisms of postoperative disturbances of motor-evacuational function of the intestine, development of early diagnosis methods, effective prevention and treatment; methods - clinical, biochemical, immunoenzymatic, instrumental, genetics, statistics; results - it was firstly researched pecularities of motor-evacuational function disorders of the intestine in the postoperative period depending on the concentration of serotonin in the blood plasma, it was found the dependence between concentration of serotonin in the blood plasma and SERT gene variants, it was firstly proven connection between unfavorable variants (SS, SL) of the SERT gene and development of postoperative disorders of the intestine contractile ability, the algorithm of postoperative intestine disorders predicting was developed the on the results of genetic studies, scientifically grounded method of diagnosis of contractile ability of the intestine through quantify the parameters of peristaltic waves and between peristaltic period, reveals new patterns of pathogenesis of disorders of intestine contractile ability, showed values in these processes imbalance of hemostasis, unlimited proteolysis, imbalance in the redox system, scientifically grounded treatment tactics in patients with postoperative paresis of intestine, ileus and ileus complicated by widespread peritonitis.
