Essay

The object - early post-operational inflammatory-destructive intra-abdominal complications; the aim - consists in the increase of the efficiency of prophylaxis and treatment of the early inflammatory-destructive intra-abdominal complications after the operations on the abdominal cavity organs by means of elaboration of the informative criteria of diagnostics, effective methods of prophylaxis and treatment on the basis of extended study of their development mechanisms; methods - general clinical, laboratory, biochemical, histological, histochemical, immune-enzyme, spectrophotometric, photoluminescentic, mathematical, statistic-analytical; results - for the first time was realized the complex comparative assessment of the dynamics of proteolytic and fibrinolytic activity, redox-reactions of the blood plasma and wall tissues of the hollow organs of digestion, levels of pro- and anti-inflammatory cytokines, circulating immune complexes in the dynamics of development of inflammatory-destructive process in the peritoneal cavity, was investigated the possibility of usage and was demonstrated the efficiency of the new method of assessment of viability of the hollow organs of digestion, was realized the pathogenetic argumentation and was investigated the efficiency of the new method of prophylaxis of the stitches failure on the hollow organs of digestion, which is based on the local inhibition of reactions of proteolysis and fibrinolysis, that allows to make 1,4 times shorter the period of formation of the connective tissue in the connection area, were studied the characteristics of the new method of junction of the walls of the hollow organs of digestion, was shown that its hemostatic capacity and physical hermeticity are, on the average, 1,2 times higher than the same of incessant and separate nodal stitches, was offered the new method of prognostication of the development of the early post-operational inflammatory-destructive intra-abdominal complications by the mathematical method, that allows to heighten the accuracy of prognostication almost to 96%
