The scientific work is devoted to the assessment of the hemostatic system during obstetric bleeding disorders caused by premature detachment of the placenta, using the method of low-frequency piezoelectric thromboelastography; optimization of the use of inhibitors of fibrinolysis in this disease. Based on the results obtained it was proved that the pathology of premature detachment of the placenta accompanied by pronounced activation of fibrinolysis. This necessitates the use of inhibitors of fibrinolysis - tranexamic acid in doses of 15 mg / kg or aprotinin 300 000 KIU (kallikreininactivating units) intravenously. It is shown that tranexamic acid produces only antifibrinolytic effect, so its empirical use is possible. Aprotinin also affects the plasma hemostatic link, so its application is necessary to monitor the status of hemostasis by thromboelastography or coahulography. It is proved that tranexamic acid has an advantage over aprotinin in reduced dosage as to inhibition of fibrinolysis.
