The paper conducts an analysis, scientific and theoretical substantiation and solving of the scientific problem of surgical treatment of great saphenous vein (GSV) acute varicothrombophlebitis (AVTP) depending on the prevalence of the pathological process, venous hemodynamics, disease duration and comorbidity. 238 (90.2%) of the patients underwent one-stage radical phlebectomy. In other 26 (9.8%) cases proximal-distal sclera obliteration, phlebo centesis + thrombectomy + sclerotherapy were conducted. The urgency of surgical intervention in GSV AVTP is determined by PE, that is why emergency surgeries have to be performed within 4 to 6 hours from the moment of patient's admission, urgent ones - within 24 hours, and postponed - more than 24 hours from the moment of patient's admission. Patients with GSV AVTP with embologenic forms of varicothrombophlebitis require immediate emergency intervention. Urgent surgical interventions are observed in localization of the proximal thrombosis level on the level of SFJ and the contact of vertical reflux with the tip of GSV blood clod. Postponed surgical interventions are observed in atypical forms of GSV AVTP, except for cases where the Leonardo vein is involved in the pathological process and the absence of contact between the reflux and the tip of the blood clod.
