The study highlights the issues of optimization of varicose disease of the lower extremities surgery with the combination withEVLC and sclerotherapy, transcutaneous laser coagulation and miniflebectomy. Use of 1470 nm laser with energy density 30 J/cm achieves irreversible veins obliteration in the early stages and minimizes the number of local complications with a corresponding improvement in the quality of life of the patients. Optimal differentiated strategies were proposed in treatment of patients with small varicose veins: sclerotherapy is preferred with a diameter of 2.0 - 4.0 mm and TLC - with a diameter of 0.5 to 2 mm. A comprehensive treatment of varicose disease involves minimally invasive laser coagulation with the application of low-energy mode laser with 1470 nm, power 10 W and average energy density 30 J/cm, estimated traction speed 3 mm/sec into the vessel, which is combined with sclerotherapy of reticular veins, TLC of telangiectases and complemented by the need with miniflebectomy.
