Dissertation is devoted to solution of the actual problem of Pediatrics - improving the treatment of acute obstructive bronchitis in infants by optimizing the anti-inflammatory therapy based on studying the molecular mechanisms of inflammation. The development of an active inflammatory process with the formation of Th2-associated response has been found during obstructive bronchitis in infants. The expression of NF-kB is the leading factor which determines cytokine profile of patients and severity of the disease. The impact of transcription factor NF-kB on proinflammatory cytokines content disappears under the influence of either systemic corticosteroids or inhaled corticosteroids. During treatment with using of systemic corticosteroids IL-13 suppresses the activity of the transcription factor NF-kB. Inhaled corticosteroids inhibit the activity of pro-inflammatory intracellular cascades by breaking the bonds between the content of the main participants of inflammation. So the respiratory disorders are managed faster during treatment with using inhaled corticosteroids. High clinical and immunogenetic efficiency has proved the feasibility of their using in the treatment of acute obstructive bronchitis in infants as the drugs of pathogenetic therapy. A mathematical model for prediction of bronchial obstruction development during acute bronchitis in infants has been worked out. It allows to prescribe a rational therapy in time.
