Haponov I. Diagnostics and therapeutic approach to diverticular disease of colon with complications

Features of clinical manifestation, diagnostics and treatment of diverticular disease of colon with complications within the regional proctologic department have been represented in the work. Frequency of complications and informational content of modern methods for diagnostics of colonic hemorrhages and purulent-inflammatory complications have been determined. Advantages of non-invasive methods of diagnostics namely ultrasound, computer control, virtual colonoscopy have been revealed, it makes possible to make conclusion about the spread of diverticula and volumes of resection of colon.Evaluation of possible usage of pro- and anti-inflammatory cytokines indices and their ratio as well as fecal calprotectin and reactivity cell and intoxication indices for the development of complications has been carried out; and their correlational connections in patients with complications of diverticular disease of colon have been verified.The diagnostic-treatment algorithm has been developed and introduced into practice. The algorithm considers diverticular disease of colon, its application made possible to obtain positive treatment results of patients with colon hemorrhages in the main group 96.05% patients versus 84.21% in the comparison group, to decrease the post-operative lethality at purulent-inflammatory complications of diverticular disease of colon from 27.27% up to 13.51%, including resections – up to 4.55%.
