The Thesis reviews data on integrated clinical, laboratory, instrumental and biochemical examination of 130 patients with arterial hypertension (AH), II stage. The main group included patients with AH without coronary heart disease (CHD), mean age was 56,5 (52,0; 62,0) years; 50 patients - with AH with stable CHD, angina pectoris, 1-2 functional class mean age was 59,5 (55,0; 63,0) years. The role of immuno-inflammatory factors (HF-CRP, IL-1beta, IL-10) associated with the pressor neurohumoral factors of the sympathoadrenal systems in the course of CHD on the background of the AH was defined. The features of left ventricular and elastic and muscular vessels remodeling in patients with AH on the background of immune-inflammatory changes were revealed. Taking into account the immune-inflammatory activation, the criteria for prediction the CHD incidence among patients with AH were worked out. There were developed criteria for prescription of antihypertensive therapy for AH patients according to individual level of activation of sympathoadrenal system.
