The dissertation is devoted to the topic of thoracic surgery to improve the treatment of patients with acute empyema by the warning of massive adhesions in the pleural cavity and by the using of mini-invasive methods of surgery. The dissertation is based on experimental and morphological researches. The features of the formation of adhesions in the pleural cavity were researched. The applying of SHF-irradiation of pleural cavity depending on the duration of the disease to prevent it was developed. The efficiency of SHF-irradiation of pleural cavity was clinically proved. Since 1st day after the punction of purulent focus we used the local SHF-irradiation to its projection on the chest with a frequency of 2,45 GHz by the "Luch SMV 3". The temperature of the procedure should be between 38,5 ± 1 ° C during 10 minutes. This mode radiation has anti-inflammatory properties and it promotes the inhibition the formation of pleural adhesions by influencing the processes of collagenogenesis. The developed techniques thoracoscopic surgery depending on the nature of pleural adhesions and using the projection's minithoracotomy can reduce the amount of thoracotomies in patients with acute empyema.
