Essay

The work is dedicated to the improvement the treatment efficacy of patients with diabetes mellitus (DM) type 2 and non-alcoholic fatty liver disease (NAFLD) through the development and implementation of a corrective treatment based on study of pathogenic relationship of general and local immunity and intestinal microbiocenosis. According to the obtained results at patients with DM type 2 and NAFLD there were recorded profound changes in the composition of the intestinal microflora. In the presence of metabolic disorders and intestinal dysbiosis at patients with DM type 2 and NAFLD there were detected the unidirectional cell-humoral immune responses in the form of inhibition of T-link immunity and activation of the immune system, increased levels of TNF-alfa, INF-gamma, IL-6, IL-8, IL-1beta, IL-2, IL-17. There was revealed the expression of endogenous intoxication markers: the molecule of average weight, the level of antibody IgG to LPS and circulating immune complexes. Increased levels of inflammatory cytokines (TNF-alfa, IL-6, IL-8) locally in the intestine correlated with their levels in the blood serum, levels of antibody IgG to LPS, fractions of average molecular weight and the degree of dysbiotic disorders of the intestine. The suggested pathogenetic grounded method of treatment of DM type 2 and NAFLD showed its clinical efficacy at 68, 8 % of patients
