Essay

The scientific work is devoted to theoretical generalization and clinical substantiation of directions to clarify the pathogenic mechanisms of headache of various genesis, its diagnosis and treatment. There was revealed a paradoxical reaction of brain vessels in patients with chronic tention-type headache and chronic posttraumatic headache, which was expressed in a perverse reaction of vessels to hypercapnia and/or hypocapnia and manifested in the reduction or lack of LVB in response to hypercapnia, and increase of LVB in response to hypocapnia. The reactivity coefficients on the hypocapnic and hypercapnic loads in patients in both groups were lower than in the patients in control group. The data were obtained when evaluating the changes of life quality in primary and secondary headaches. It was revealed that the most significant changes in quality of life parameters are defined on a scale of pain intensity, vitality, emotional functioning and social functioning. Based on these data the algorithm of complex treatment of disorders of reactivity of cerebral vessels in patients with chronic post-traumatic headache, сhronic tension-type headache was worked out.
