Parfentyeva N. Functional plastic of incisional ventral hernias

The research is based on the results of surgical treatment of ventral hernias in 280 patients with Incisional ventral Hernias. Patients were divided into two equal groups: control group of patients whose surgery was performed by the "onlay" method and the main group, patients whose surgery was performed by the original method, which included the opening of the anterior walls of mm. recti vaginae, with construction to direct muscle midline, followed by aloplasty with synthetic implant. The original method is simple and less traumatic, it allowed 4,1 times reduce the number of relapses, 6 times lower incidence of seromae. The investigation of function of mm. recti abdominalis in the postoperative period allowed to prove that developed original method provides a better functional outcome index "Abdominal Wall Strength" according to special tests in the study group after the operation amounted to 7,3 ± 0,2 points, and in the control group - 4 0 ± 0,2 points. According to Electromyography in patients of the main group almost completely recovered function of mm. recti confirming the average amplitude tour in Electromyography, which is equal to 1195 ± 55.1 mcV, and the average frequency of the tour, which was 121,2 ± 6,7 Hz; in the control group, the mean amplitude Turnu was 788,8 ± 89,2 mcV, and the average frequency 111,5 ± 9,9 Hz. Quality of life on a scale HerQLS in the study group amounted to 44,4 ± 2,4 points, and in the control group 35,1 ± 2,5 points, indicating that the better results in the study group.
