Essay

The thesis is devoted to the improvement of approaches to diagnosing and treatment of coronary heart disease comorbid with hypothyroidism based on the detection of clinical and pathogenetic role of neurohumoral and immunoinflammatory disorders. It has been found out, that coronary heart disease in patients with comorbid hypothyroidism occurs with significantly greater severity of ischemic and vegetative changes, the manifestations of its electrical instability and cardiac remodeling accompanied with intensification of systemic inflammation and endothelial dysfunction against the background. On the basis of correlation and regression analysis the cognitive model of relationships between markers of systemic inflammation and endothelial dysfunction and indicators of cardiac remodeling, ectopic activity of the heart, coronary and autonomic disorders, depending on thyroid function, where the FT4 acts as a connecting link was built. It was proved that the use of quercetin against the background of standard treatment in patients with coronary heart disease and hypothyroidism is accompanied by a reliable decrease in myocardial stiffness and improvement of left ventricular diastolic function, decrease of the severity of myocardial ischemia and arrhythmic complications, decreased activity of systemic inflammation and endothelial dysfunction.
