Suprun O. Substantiation of an active surgical approach in patients with degloving and combined injuries of the limbs (a clinical and experimental study).

The dissertation is an experimental and clinical study. The study implied experimental determination of terms and identification of intervals characterizing the state of viability of injured tissues of the patient. The experimental results were confirmed by morphological studies. The study involved clinical examination of 221 patients with degloving and combined injuries of the limbs who were divided into two groups: the comparison group comprising 124 patients and the main group with 97 patients. The findings provided evidence allowing to substantiate the types and extent of preventive and early conservative surgery in patients with degloving and combined injuries of the limbs with introduction of modern achievements of reconstructive-rehabilitation surgery. Development and implementation into clinical practice of an algorithm of operational and tactical methods of early surgical treatment of patients with degloving and combined injuries of the limbs made it possible to shorten the preoperative period by 35.5%, the term of inpatient treatment by 13 hospital days, the number of complications reduced two-fold, the number of surgical interventions per patient decreased by 1.6 times.
