The work deals with the improvement of the diagnostics and effectiveness of therapy of women with chronic salpingo-oophoritis based on the study of immunological and microbiological features of the organism, namely the formation of extracellular neutrophil traps and the ability of microorganisms to form biofilms. On the ground of the studies including 105 women comprehensive assessment of the reproductive health in women with chronic salpingo-oophoritis is presented. Comparing phagocytic activity of unchanged neutrophils and the efficiency of bacterial capturing in neutrophil traps in cases of chronic salpingo-oophoritis it is found that the neutrophil traps, which are located extracellularly, are able to capture more antigens than a neutrophil itself. The ability of biofilm formation by microorganisms was studied, what allowed to form a novel approach to the administration of antimicrobial therapy and to full characteristics of the mechanisms contributing to the development of the inflammatory reaction. The lines of evidence on the structural and functional state of the immune system and cytokine status in chronic salpingo-oophoritis allowed to ground a conceptual model of immunopathological mechanisms of the development of immunological disorders and nonspecific resistance. The analysis of the obtained results shows that the therapy of patients with chronic salpingo-oophoritis on the proposed scheme with immunomodulating therapy leads to the restoration of functions of both local and general immunity. The administration of the combined therapy in case of chronic salpingo-oophoritis is found to be the most rational and effective.
