Goni S. Optimization of surgical treatment of patients with chronic critical ischemia of the lower extremities with distal form of vascular lesion.

In this work, the results of treatment of 105 patients with chronic critical ischemia of the lower limbs were determined and analyzed, and the methods of treatment according to the indications were applied. Of these, 55 patients underwent stimulation of neoanogeogenesis by of platelet-rich autoplasma. A method for stimulating of neoangiogenesis with the platelets rich autoplasma, which was obtained immediately before injection by soft centrifugation, was developed and introduced. The analysis of the immediate and long-term results of treatment (dynamics of quality of life of patients, level of pain, distances of painless walk, level of ET1, VEGF, CRP, fibrinogen in blood plasma, number of small and large amputations) showed that in long term stage after treatment (12 months) patients which were treated with a conservative method with plasma therapy had a significantly higher quality of life compared with patients treated with a conservative method without plasmotherapy.
