Dissertation is devoted to the choice of the optimal scheme of antithrombotic therapy problem in ischemic heart disease in combination with atrial fibrillation patients by investigation the pathogenetic role of erythrocytes and platelets phospholipid membranes fatty acid spectrum changes in the mechanism of various atrial fibrillation clinical forms development. Fatty acid spectrum features of blood erythrocytes and platelets membranes phospholipids in ischemic heart disease (IHD) without cardiac rhythm disorders and in combination with various clinical forms of atrial fibrillation (AF) patients have been established. Fatty acid spectrum changes of red blood cells and platelets membranes phospholipids have been studied in the background of the sinus rhythm restoration. Correlation between supraventricular extrasystoles number and fatty acid spectrum of platelet membranes phospholipids in ICH with paroxysmal and persistent AF forms after restoring sinus rhythm patients with have been revealed. It is shown that in IHD in combination with paroxysmal and persistent AF clinical forms patients it is expedient to prescribe a regimen of drug therapy that includes the rivaroxaban use, and IHD in combination with a permanent AF form patients – the appointment of a pharmacotherapy scheme including warfarin. Also in patients with IHD with AF with an initial increase in the relative content of palmitic and oleic acids, a decrease in stearic acid in erythrocyte membranes phospholipids and linolenic acid in platelet membranes phospholipids – rivaroxaban is advisable because it is associated with a decrease AF paroxysm duration arachidonic acid relative content in the platelet membranes phospholipids.
