This thesis is dedicated to improvement of direct and remote results a year after surgical treatment of patients with cases of combined diseases of abdominal cavity and ptosis of its anterior wall by developing new methods of simultaneous laparoscopic intervention and optimization of surgical process. Depending on the technique of simultaneous surgeries (open, traditional laparoscopic, improved laparoscopic), the patients were divided in three groups. Comparison group I (n = 74) consisted of patients who, after dermolipectomy, underwent open simultaneous surgeries. In comparison group II (n = 52) patients first received laparoscopic intervention in organs of abdominal cavity with percutaneous insertion of trocars in abdominal cavity (simultaneous stage), after which dermolipectomy was performed (main stage). In the main group (n=50) the skin-fat flap is dissected at first. The flap isn't cut off but fixed to a horizontal arm to perform laparolifting, which allows the laparoscopic intervention at carboxy peritoneum 6-8 mmHg. Trocars were inserted directly through the aponeuro-muscular layer underneath the dissected skin-fat flap. Evaluation of results was based on the number and character of perioperative complications which arose, and also on the complications in a remote postsurgical period 12 months after the surgery and quality of life of the patient 12 months after disease. The survey was conducted using the questionnaire SF-36 ("SF-36 Health Status Survey). Comparative analysis of surgical treatment of patients with combined diseases of abdominal cavity and ptosis of the anterior abdominal wall using open, standard and improved laparoscopic methods showed, that using the improved laparoscopic technique has decreased the number of postoperative complications related to cardiovascular and respiratory systems from 17.6% and 22.9% respectively to 4.0%, early local postoperative complications from 20,0% and 25.0% to 6.0%, remote one year after surgery complications with 20,8% and 25,0 % to 7,2%; improve the average score of the physical health component with79,61 ± 0,94 and 81,43 ± 0,91 to 87,89 ± 1,03 score, and the average score of the psychological component of health with 80,60 ± 0,96 and 81,86 ± 1,15 to 86,25 ± 1,23 score.
