Essay

The dissertation presents the theoretical grounding and there has been achieved a new practical solution of an actual scientific problem in the field of clinical medicine, namely in the field of cardiology, concerning the definition of the mechanisms of the development of post-implantation atrial fibrillation in ischemic heart disease and the increase of the effectiveness of the treatment and prophylaxis of paroxysms in older patients. There were 387 patients under the survey. Based on the complex of clinical, laboratory and instrumental studies the state of the cardiovascular system has been assessed, the pathophysiological and prognostic role of systemic inflammatory markers, coagulation-aggregation parameters, hyperoxidation, endothelial function of blood vessels, lipid metabolic factors and myocardial remodeling markers in the appearance and support of paroxysmal form of atrial fibrillation and its complications have been proved, their dynamics by the usage of two therapeutic regimens (standard generally recognized medicinal and the one with adding of liposomal forms of phosphatidylcholine and quercetin) has been assessed, direct and long-term results, the frequency of the breaking up of paroxysmal activity, annual survival and the quality of older patient's life with post-implantation atrial fibrillation have been evaluated
