Kondratenko I. Optimization of diagnostics and tactics of conducting pregnancy at women with disturbances of volume of amniotic waters.

Dissertation work is devoted to depression of perinatal complications at pregnant women with pathological volume of amniotic waters on the basis of studying of clinical-functional, endocrinologic, microbiological, morphological features of condition of fetoplacental complex and development of advanced algorithm of treatment-and-prophylactic actions. Need of improvement of algorithm of diagnostics for prophylaxis of perinatal complications at women with disturbances of volume of amniotic waters is scientifically reasonable. Features of a functional condition of fetoplacental complex at women with hydramnion (sclerosis and stenosis, disturbances of vascularization of vessels and stroma of villiferous tree) and with an oligoamnios (the multiple centers of infarcts, hemorrhages, narrowing of intervillous space) are established. Correlation of level of alpha-fetoprotein with severity of complications at the initial stage of development of placental dysfunction is shown (at combination of dystrophic changes in placenta to fetus hypoxia) synthesis alpha fetoprotein in fetus liver compensatione amplifies that is confirmed by augmentation of frequency of high values of this indicator in blood of mother (to 48%). At fetus arrest of development in 63% of cases low level of an alpha-fetoprotein in blood of mother that can be consequence of subcompensation of biochemical processes in hepatic tissue of a fetus against the expressed morfofunctional disturbances of placenta takes place.
