The thesis contains the justification of theoretical, methodological and practical basis on organizational mechanism of public administration in palliative and hospice care sphere, as well as the directions of this mechanism improvement in Ukraine.
It was ascertained that there is no holistic studying on the organizational mechanism of public administration in palliative and hospice care in Ukrainian Public Administration Science. It was suggested and justified the author’s definitions of the concepts of ‘public administration in palliative and hospice care’ and ‘organizational mechanism of public administration in palliative and hospice care’. The principles of palliative and hospice care delivering are scientifically justified. The aim, objectives of the state’s impact on the palliative and hospice care, the subjects and objects of the public administration in the care are determined and characterized.
It has been identified that the contemporary palliative and hospice care elements have taken place in the early civilizations, such as the sympathy of society, the humane treatment of people with incurable diseases, the care delivering and the ensuring of the decent conditions for the dying. It was disclosed the differences in historical meaning and the social role of the hospice that are embodied in the human behavior culture to deadly patients and are transmitted invariably through the ages from the ancient to the present generations. It is emphasized on the need to create the decent conditions for quality of life for incurable patients in Ukraine.
Based on the PESTL-analysis there were determined and examined the up-to-date factors that impact on the public administration in palliative and hospice care in Ukraine, namely, political, economic, socio-demographic, technological and innovative, normative and legal, institutional and institutional.
The studying of the foreign experience revealed different approaches to the application of the organizational mechanism of public administration in palliative and hospice care. It was distinguished the Western countries with the high levels development of palliative and hospice care, in particular the United Kingdom, Ireland, Belgium, Germany, the Netherlands, France and Poland. In the countries, considerable attention is given to the pediatric hospice care development. The ‘traditional’ and the current models of the organizational mechanism of public administration in palliative and hospice care are determined and characterized.
The synergistic integration paradigm is submitted and scientifically justified, which is considered as the imperative to improve the organizational mechanism of public administration in palliative and hospice care. The directions and practical recommendations for improving the system of palliative and hospice care through the organizational mechanism of public administration are defined and justified.
