The object of the study – acne. The aim – to increase the effectiveness of treatment of patients with moderate and severe acne by clarifying indications for the appointment of systemic retinoids, combining them with systemic and topical antibacterial agents, and also the means of preventing the expected side effects based on in-depth study of action mechanisms of systemic retinoids. Methods: clinical, microbiological, immunohistochemical, pathomorphological, clinical and instrumental, statistical. Based on the comparative analysis of clinical manifestations of acne, assessment of microbiocenosis of the skin surface, laboratory studies, epiluminescence method data, pathomorphological, immunohistochemical and psychometric methods, various types of pathological changes in the microbiome, skin structure of acne patients have been determined. Based on the research, three groups of patients with acute forms of acne have been formed, in which different types of therapy with the combination of systemic retinoid and systemic and topical antibacterial agents have been applied. Pathomorphological and immunohistochemical aspects of acne pathogenesis in patients with and before treatment with systemic retinoids were studied. It is established that the proposed combinations have different periods of regression of acute lesions of acne and increase the clinical effectiveness of therapy.
