Huseynov A. Surgical treatment of comminuted fractures of a mandible

The thesis is devoted to the urgent problem of a surgical dentistry – treatment of patients with comminuted fractures of a mandible. In the thesis, the features of clinical character, surgical treatment comminuted fracture of a mandible were studied. It is framed the system of assessment of gravity prognostic character comminuted fracture of a mandible in which comminuted fractures of a mandible were divided into 3 groups: «comminuted fractures of mandible with a favorable current», «comminuted fractures of mandible with a doubtful current», «comminuted fractures of mandible with an adverse current». The found features prognostically of character gave the chance to prognosticate results of treatment comminuted fracture of a mandible when entering. Treatment of patients of group 1 was performed conservatively and operative(open reduction and fixation of the mandible with using of mini plates), 2 – operatively (except 1 patient, whose treatment was conservatively – mandibulomaxillary) (with preservation of bone fragments, open reduction and fixation of the mandible), 3 – open reduction and fixation of mandible with the removing of freely lying bone fragments and with using of bone reconstructive plates according to the requirements of biomechanic. The complication of fractures in the I group wasn’t observed, in IІ group were 2 (6 %), in III to group 2 (20 %). It is taped that in all cases comminuted fractures of a mandible disturbance of a circulation is observed. At the same time quantitative indices approach normal values of the rheographic index in the second and third clinical groups 21 days after surgical intervention in time.
