MARTYNIUK D. Clinical-pathogenic validation of methods of treatment of anterior optical ischemic neuropathy.

The objective of the dissertation is improvement of treatment efficiency of patients with АION by means of learning the influence of direct neuroprotection, correction gradient of nocturnal hypotension and optic nerve perfusion pressure on АION progress, depending on arterial pressure rate. The scientific knowledge about features of daily fluctuation of arterial pressure of АION patients has been significantly broadened. It was determined, that in all groups of patients the minimal indexes of average AP and HR were registered during the same time period, from 22.00 till 24.00. Also, we have updated the scientific knowledge about the key features of daily fluctuation of intraocular pressure of АION patients, depending on AP rate, explored the rates of perfusion pressure (PP) and its fluctuation, depending on AP rate (hypertension disease (HD), arterial hypotension and normotension), proved, that in group with patients with АION and arterial hypotension, the average rates of PP were the smallest, compared to groups with normotension and HD of I and II stages. The complex treatment scheme was developed for АION patients, with utilization of correction of PP and AP rates. It was discovered, that the correction of nocturnal PP for АION patients was reached by decreasing intraocular pressure (brimonidine tartrate 0.15% one drop two times/day. The scientific information has been extended and have been given the results of researches of morphometric and functional indexes of optic nerve for АION patients, depending on PP rate.
