Essay

The object of the study - rosacea in women. The aim - to increase the efficiency of treatment of patients with rosacea in women by developing complex differential therapy based on the study of peculiarities of the mechanism of rosacea development, clinical morphological manifestations and the state of the vascular bed with the use of means and techniques that normalize the level of vascular endothelial growth factor and vascularity. Methods: clinical, anamnestic, instrumental, morphological, immunohistochemical, assessment of life quality and psychosomatic status, statistical. The dissertation is devoted to increase of efficiency of complex differentiated therapy of women, patients with rosacea, on the basis of studying features of the mechanism of development, clinical morphological manifestations and the state of the vascular bed of dermatosis. The peculiarities of the influence of pathology of the vessels of the head and neck on the course of rosacea have been investigated. The relationship between rosacea in women and increased expression of endothelial growth factor receptors (VEGF), CD34+ - endothelial marker, and estrogen / androgen receptor (AR, ER) have been established. The algorithm of examination of women with rosacea patients is suggested, which allows to find out changes that affect the nature and severity of the skin inflammatory process. The pathogenetically grounded method of complex treatment of women with rosacea patients with the use of an intensive pulsed light system (IPL) has been developed, the effectiveness of which is confirmed by reduction of treatment periods and prolongation of the remission period. The main scientific and practical results of the dissertation work were introduced into the medical practice of the "Institute of Psoriasis and Chronic Dermatoses Ltd." (Kyiv), "Institute of Plastic Surgery "Virtus" Ltd." (Odessa), Medical Center "Asclepius" (Uzhhorod), Regional Clinical Dermatovenerologic Dispensary (Uzhhorod), MI "Regional Clinical Dermatovenerologic Dispensary" (Dnipro), MI "Dnipropetrovsk Regional Pathologoanatomic Bureau", Ternopil Regional Clinical Dermatovenerologic Dispensary, MI "Cosmetological Hospital" of Dnipro City Council, as well as in the educational process of the department of dermatovenereology of the National Medical Academy of Postgraduate Education named after P.L. Shupyk, department of skin and venereal diseases with courses of pathomorphology and phthysiology of the SHEI "Uzhgorod National University". Area of usage - dermatovenereology.
