Essay

The current dissertation presents the results of clinical and experimental study of 104 patients with primary unilateral inguinal hernia who underwent a modified laparoscopic transabdominal preperitoneal hernioplasty (main group, 56 patients) or traditional open Lichtenstein repair (control group, 48 patients). On laboratory animals (rabbits), the methods for closing the peritoneal defect over the mesh implant - welding and ligating were evaluated in terms of repair quality and safety. Factors that most significantly influenced the development of inguinal pain after surgery and the recurrence of hernia in the named groups were studied. The hospital stay was reduced by 3 times and the period of the restoration of ability to perform professional duties was 3.4 times shorter when using the modified TAPP method in comparison with the control group. Thus, the obtained results prove the effectiveness of the use of modified laparoscopic transabdominal preperitoneal hernia repair technique for primary unilateral inguinal hernias in comparison with the traditional open Liechtenstein method with regard to the reduction of the operation time, length of stay and restoration of the ability to perform professional duties, as well as to the reduction of postoperative pain risk development, perioperative complication and recurrence rates.
