Essay

The work is devoted to the improvement of the results of treatment of patients with acute biliary pancreatitis and its complications - obstructive jaundice, isolated centers of necrosis, pancreatic pseudocysts and chronic pancreatitis with duct hypertension, by determination of indications, improvement of the technical methods and evaluation of the results of the use of endoscopic transenteral and combined transpapillary interventions on the common bile duct and pancreatic duct. There were developed and improved technical methods of trans-duodenal access to the common bile duct in acute biliary pancreatitis. The efficiency of transenteral (through a stomach and duodenum) endoscopic interventions and stages endoscopic pancreato-sequestrectomy in isolated foci of necrosis has been set. The indications for transenteral and combined transpapillary interventions in patients with isolated foci of necrosis and post-necrotic cysts of the pancreas were determined. The indications for endoscopic transpapillary interventions in patients with chronic pancreatitis with duct hypertension, namely, in lithiasis of Wirsung's duct, post-necrotic cysts, which are connected with the pancreatic duct, strictures of the pancreatic duct, as well as internal pancreatic fistulas is determined. Early and late complications of these interventions have been studied; measures for their elimination have been worked out The effectiveness of endoscopic treatment of patients with chronic pancreatitis with the duct hypertension was studied
