The scientific work is devoted to the study of retrospective and prospective aspects of the features of preterm labor and their perinatal consequences in pregnant women, the identification of risk factors for possible preterm births and the allocation of a risk group, the study of basic clinical, echographic and laboratory changes in them, as well as the improvement of the algorithm of diagnostic and therapeutic and prophylactic measures. Based on a retrospective analysis, the main anamnestic factors that were most commonly observed in pregnant women with preterm labor were identified. This allowed us to regard them as risk factors for premature labor in pregnant women and to form a risk group. In a further prospective study of pregnant women at risk, the role of fetal fibronectin and interleukin-8 in the study of serum and cervical mucus in the complex was determined. In parallel, cervicometry and vaginal microbiocinosis were evaluated. Correlation analysis was carried out. The results of the study showed that the definition of fetal fibronectin and interleukin-8 in the complex while simultaneously studying them in blood serum and cervical mucus in a group of women threatening anamnestic and clinically related to possible preterm labor significantly increases diagnostic capabilities and provides the opportunity in a timely manner to the occurrence of shortening of the cervix uterus and the appearance of a clinical picture to direct a pregnant woman to a hospital for prophylactic treatment. The obtained results allowed us to scientifically substantiate the necessity of improving the algorithm for the management of pregnant women at risk for the occurrence of preterm labor. The rational tactics of diagnostic and treatment-preventive measures for the management of pregnant women at risk for preterm labor is developed.
