Essay

The object of research: type 2 diabetes with different types of isolated dyslipidemia. The aim of the study: The investigation of the fatty acid composition of serum lipid fractions of T2DM patients with different types of isolated dyslipidemia and justification of differentiated approaches to their pharmacological correction. Methods of research: general-clinical, anthropometric, biochemical, chromatographic and statistical methods. Theoretical and practical results: The expediency of monitoring of fatty acid composition of separate lipid fractions of blood serum of patients with diabetes mellitus type 2 with isolated forms of dyslipidemia has been proved. There was proved that visceral obesity index could be used as a predictor of the possibility of a violation of the fatty acid spectrum of lipid fractions in the general group of T2DM patients. The lipid-lowering therapy in the correction of lipid disorders in patients with isolated dyslipidemia based on revealed changes in the fatty acid spectrum has been suggested. Novelty: For the first time, a comparison was made between the features of the lipid profile and the fatty acid spectrum of lipid fractions of blood serum of T2DM patients with different types of isolated dyslipidemia. For the first time, it was established that the visceral obesity index shows a direct correlation with the degree of saturation of fatty acids of fractions of TG and FFA and the inverse - with the level of all unsaturated FA of the fraction of PL. For the first time, it has been shown that the use ω3-PUFA for the treatment of patients with isolated hypoalphalipoproteinemia can increase the level of oleic acid and the essential ω3-PUFA in the fractions of FFA and PL. There was shown for the first time decreasing of saturation of all lipid fractions of blood serum of T2DM patients with isolated hypertriglyceridemia because of fenofibrate therapy. New data on the possibility of reducing the ratio between the content of linoleic and arachidonic acids in blood serum of T2DM patients with isolated hypercholesterolemia using the inhibitor of the enzyme HMG-CoA reductase were obtained.
