18-33% of patients with mitral insufficiency and preserved left ventricle ejection fraction before surgery demonstrated left ventricular dysfunction after surgical intervention on the mitral valve. The left ventricle ejection fraction ≤40% after surgical correction of mitral insufficiency leads to an increased risk of late postoperative death to 70%. After the plasty or replacement of the mitral valve the preload of the left venatricle decreases, afterloading increases and it may disrupt its systolic dysfunction. The risk of late postoperative mortality in patients with decreased left ventricular preoperative systolic function is significantly higher than in patients with preserved one, and its objective estimation in such patients by standard methods is inaccurate, which became the basis for this study. The performed study allows to recommend the method of an effective ejection fraction calculation as a simple and easy-reproducible additional echocardiographic method for assessing the left ventricular systolic function in patients with severe primary mitral regurgitation.
